All prize distribution is processed via Direct Deposit to Bank (ACH) as requested on this form.
Any cash withdrawals that total $600 or more in a single fiscal year will require a W-9 to be filled out and will generate an IRS Form-1099 for the year in which the withdrawals are paid.
Any funds left in your account can be used for next year's league registration. Only funds which are requested for withdrawal are subject to filling a W-9 form.

I hereby authorize Guillotine Leagues (Heavy Blade, LLC) to electronically credit my account and, if necessary, to electronically debit my account to correct erroneous credits at the depository financial institution named below/above. I agree that ACH transactions I authorize comply with all applicable law. I understand that this authorization will remain in full force and effect until I notify Guillotine Leagues (Heavy Blade, LLC) in writing or by phone that I wish to revoke this authorization. I understand that Guillotine Leagues (Heavy Blade, LLC) requires at least (2) days prior notice in order to cancel this authorization.

Withdrawal requests will be open for processing on Saturday, December 7th.
If your withdrawal requests for the year total $600 or more, you will be redirected to complete an IRS W-9 form. Once you submit the completed W-9 form, you MUST RETURN TO THIS PAGE to complete your withdrawal request.

Self-Exclusion

In compliance with certain US state law, if you wish to disable or permanently deactivate (close) your account for any reason, you may submit your request using the section below. By requesting to deactivate your account, the following steps will be taken:

Access to your account will be removed for the length of time which you specify.
Any league entries eligible for refund will be removed and refunded.
You may request to withdraw any remaining funds from your account.

At the end of your self-exclusion period, you may choose to open access to your account by contacting customer support at support@guillotineleagues.com.
